New Student Registration
New to Grace Yokley Jr. High? Here is a list of items that you need when registering your child:
1. Proof of Birth (Required)
- Certified copy of birth record
- Statement by the local registrar or county recorder certifying date of birth
- Baptism certificate
- Passport
- When none of the foregoing is obtainable, an affidavit of the parent, guardian, or custodian, or any other appropriate means proving the child's age.
2. Immunization Record (Required)
- A student enrolling in the Mountain View School District must present documentation from a health care provider certifying that he/she has received all immunizations required for his/her age or grade level as specified by the California Department of Public Health. Requirements are listed in the registration packet.
- If the immunization records being brought from a foreign country are in a language other than English, the parents must provide a copy of the immunization record which has been translated into English by a certified doctor. The translated copy should be written on the doctor's letterhead or have the doctor's office stamp on it which show the doctor's address and telephone number.
3. Individualized Education Plan (IEP)

4. Verification of Residence (Required)
(must be current)
- Utility Service contract, statement or payment receipt
- Property tax payment receipt
- Rental property contract, lease, or payment receipt
- Pay stub
- Voter registration
- Correspondence from a government agency
- Shared Residency Form
5. Optional
(Optional items can be very helpful in placing your child in the appropriate class or programs.)
- Last Report Card
- State Test Results
Once at our school, you will need to complete the following:
- Enrollment Form
- Home Language Survey (if new to California)
- Residency Questionnaire
- Custody Law Notification
- Student Records Request Form (Cumulative records from last school)
- Student Health Information Sheet
Other:
Inter-District Transfers* (Students living outside of our attendance boundaries). Parents/Guardians should report to the district in which they reside for an application.
