New Student Registration

• Certified copy of birth record
• Statement by the local registrar or county recorder certifying date of birth
• Baptism certificate
• Passport
• When none of the foregoing is obtainable, an affidavit of the parent, guardian, or custodian, or any other appropriate means proving the child's age.

• A student enrolling in the Mountain View School District must present documentation from a health care provider certifying that he/she has received all immunizations required for his/her age or grade level as specified by the California Department of Public Health. Requirements are listed in the registration packet.
• If the immunization records being brought from a foreign country are in a language other than English, the parents must  provide a copy of the immunization record which has been translated into English by a certified doctor. The translated copy should be written on the doctor's letterhead or have the doctor's office stamp on it which show the doctor's address and telephone number.

• Utility Service contract, statement or payment receipt
• Property tax payment receipt
• Rental property contract, lease, or payment receipt
• Pay stub
• Voter registration
• Correspondence from a government agency
• Shared Residency Form

(Optional items can be very helpful in placing your child in the appropriate class or programs.)
